She was taken to 1969 by the Silence, where they brainwashed her into wanting to kill the Doctor. They then put her in a spacesuit in order to sustain her with its life support system, and that's when she met the Doctor, Amy, Rory, and River (as seen in {{DW|The Impossible Astronaut}}). After her spacesuit is shot by Amy, she escapes it and flees. Six months later, we see her regenerating in New York ({{DW|Day of the Moon}}). She then becomes Mels, and grows up alongside Amy and Rory in the 1990s. She lives normally for a while, and in 2011, she follows Amy and Rory to a wheat field where they leave a message for the Doctor, and joins them on the TARDIS, which crash lands in 1938 Berlin due to Mels shooting the console. It is in Hitler's office that she is shot by him and regenerates in River Song ({{DW|Let's Kill Hitler}}).

No, there is no "first place." It's a closed time loop. A future Doctor always gave Rory a screwdriver to open the Pandorica, and the past Doctor always ended up becoming the future Doctor that gave Rory the screwdriver. Doctor Who has shown other instances of this before. Here is a list of some of those instances:

{{DW|The Parting of the Ways}} (and the "Bad Wolf" storyline in general) - Rose sees the words "Bad Wolf" at a nearby court, which convinces her to try prying open the Heart of the TARDIS so she can save the Doctor. She succeeds, and the Heart fills her with vortex energy. When she arrives in the future where the Doctor is, she sees the "Bad Wolf Corporation" name and scatters the words "Bad Wolf" across spacetime in order to lead herself to that moment. The "Bad Wolf" at the court and the name of the "Bad Wolf Corporation" are only there because she spread the words across spacetime, and she only spread those words around because they were what led her there.

{{DW|Smith and Jones}} - A future Doctor shows Martha his tie, which causes her to meet him before she sees a younger Doctor in the hospital. The younger Doctor and Martha then stop the Judoon, and younger Doctor offers to take Martha with him. She wants proof that he's a time traveler, so he goes back and takes off his tie and becomes the Doctor that Martha saw. Martha would have never recognized the Doctor in the hospital and asked him to go back had he not gone back and shown her his tie.

{{DW|The Doctor's Daughter}} - The TARDIS picks up on the presence of another Time Lord, and takes the Doctor, Donna, and Martha to the future where the Time Lord (Jenny) is created. Her creation then alerts the TARDIS, which takes the Doctor, Donna, and Martha to the future.

{{DW|The Lodger}} - The Doctor finds a note telling him to go to Craig's flat, and at the end of the episode, has Amy write the note and takes her back to before the episode took place so that she can leave the note in the window for his past self.

There are others, of course, but the point is that it's a never-ending cycle. It's also a minor form of a paradox (River's life is littered with paradoxes like this), but it is possible due to time travel. The Doctor was let out by Rory who had a future Doctor's screwdriver.

However with Sec the Doctor encounters a Dalek who realizes that the Daleks needed to change and is himself willing to change, and the Doctor was willing to help him. As Sek is killed by other Daleks and they did not change, the Doctor has never had a good reason to reconsider his opinion of them.

He's not erasing himself from the actual events, only from the records of those events, & we've not been given details of how he's been doing this.

Some clues have been given, however, going as far back as the Series 1 episode {{DW|World War Three}}, in which the Ninth Doctor gave Mickey Smith a disk with a computer virus that would remove references to the Doctor from electronic records.

Subsequently, in the Series 7 episode {{DW|Asylum of the Daleks}}, Oswin Oswald, one of the manifestations of Clara Oswald, hacked into the Daleks' Pathweb & removed their records of him. In a conversation between the Doctor & River Song in {{DW|The Angels Take Manhattan}}, later in Series 7, it was revealed that the Doctor had continued the process of erasing records.

John Hurt is an Oscar-nominated actor most famous for his role as Kane in the 1979 movie ''Alien'', in particular the (in)famous "chest-bursting" scene. This movie and the actor John Hurt apparently exist in the ''Doctor Who'' universe, as Toshiko Sato references the scene in {{TW|Greeks Bearing Gifts}}, specifically mentioning Hurt by name.

Behind the scenes, Hurt portrays an as yet unknown incarnation of the Doctor in {{DW|The Name of the Doctor}}. The Eleventh Doctor and Clara Oswald encounter him when the Doctor entered his own timestream to save her. Eleven said that he chose the name "the Doctor" for himself as a promise, and that this incarnation broke that promise. This forgotten incarnation, the Doctor's secret, defended his actions saying that he did them for "peace and sanity". The Eleventh Doctor didn't dispute this, but did suggest that because of what he did he could not properly call himself "the Doctor". Despite this, Hurt is directly credited on screen - twice - as the Doctor.

The core belief in the religion of the Silence is that "on the Fields of Trenzalore, at the fall of the Eleventh, when no creature can speak falsely or fail to answer, a question will be asked — a question that must never ever be answered," and that "silence will fall when the question is asked." The Silence knew that it was the Doctor who would be asked the question. It is unclear what details they knew beyond that: what the question was, what the answer was, why it must never be answered and how it would cause "silence to fall", though it does appear they knew very little beyond that prophecy. Their goal was to simply prevent the Doctor from reaching Trenzalore and being asked the question.
